Interscholastic Athletic Opportunities Disclosure Forms
As per Commonwealth of PA Law - Act 82 Article XVI-C, a school entity shall make available a hard copy of the completed Interscholastic Athletic Opportunities Disclosure Form for public inspection during regular business hours, including on any publicly accessible website of the school entity.
A hard copy of the document is available in the Administrative Offices weekdays from 8:00 AM - 4:00 PM unless school is closed due to a holiday, inclement weather, or some other uncontrollable circumstance.
A copy of both the High School Interscholastic Athletic Opportunities Disclosure Form and the Middle School Interscholastic Athletic Opportunities Disclosure Form can be found at the bottom of this page.
