Please print and complete the Application for Work Permit below. The student should bring the completed form with a copy of the student's birth certificate, passport, baptismal certificate, or other proof of birth date to either the Salisbury Middle School or Salisbury High School main office. Office personnel in either building will process the application and issue the Work Permit, regardless of which school the student attends. Students must be 14 years of age to apply for the permit.
